Australia:
Identification documents:
- Passport (valid original); if you do not have a valid passport, please ask your case officer for advice and/or consult the DHA website for acceptable alternatives.
- Health examinations list (HAP letter) with HAP ID.
Medical records:
- Any medical reports on past or current medical conditions.
- Vaccination records (optional).
- Child Health and Development Record for children, if available.
Canada:
Identification documents
- Passport (valid original or photocopy certified by a Canadian Visa Office) or a valid national ID card (an original birth certificate is an acceptable national ID document for minors under the age of consent only).
- Medical Report Form (IMM 1017) with client identifiers (IME or UCI number), except for those presenting for an Upfront Medical Exam (UFM). Following completion of the health assessment, UFM applicants will be issued a document generated from eMedical as proof of compliance with the medical requirements.
