What to bring on the day of the appointment	Australia: (Ukrainian, English) Health examinations list (HAP Letter) with HAP ID International passport (valid original)  Medical records about past or current severe/chronic diseases/surgeries (if any) A separate printed document that contains: Email address and personal phone number Address in Ukraine in English. Canada: (Ukrainian, English) Medical Report Form from the Embassy (For visa applicants who have already submitted their application to the Embassy, for the Canada-Ukraine Authorization for Emergency Travel (CUAET) visa, the medical form from the Embassy is not required) International passport (Or Ukrainian passport if the visa applicant has a medical form from the Embassy and the international passport is at the Embassy) Medical records about past or current severe/chronic diseases/surgeries (if any) COVID-19 vaccination certificate (indicating all doses received, if applicable) A separate printed document that contains: Email address and personal phone number Address in Ukraine in English.
