- Health Examination List letter/referral letter from the Embassy
- Any medical reports or test results for any previous or existing medical conditions (if applicable)
- Vaccination record, including COVID-19 vaccination certificate
- Proof of payment
Canada:
- Original passport
- Medical report form (IMM1017)
- Any medical reports or test results for any previous or existing medical conditions and a list of current medications (if applicable)
- Vaccination record, including COVID-19 vaccination certificate
- Eye glasses or contact lenses (if applicable)
- Proof of payment
New Zealand:
- Original passport
- Health Examination List letter/referral letter from the Embassy
